You can create with doPDF PDFs that are larger, as long as the program you're printing from has support for that. But just to be clear, you'd print to PDF an image/document with a map, not a PDF to a PDF.

Please make sure that you've selected doPDF as the printer when printing that invoice. Regarding the feature suggestion, we'll consider it for a future release.

We didn't have reports until now of incompatibilities with Revit 2016, so it must be something very specific. doPDF doesn't have a "Start with Windows" option, so something must trigger the window (maybe you have a pooled print?). I'd suggest reinstalling doPDF to be sure everything is cleaned.

You will not be able to use the Getting Started window if you only keep the printer driver. What version of doPDF are you using? The Getting Started window should only start on request (i.e when you search for and open doPDF) and there's no option to start it each time.
