Yes it's possible, when you select doPDF as a printer in Visio, go to its Properties and on the Page tab there's a way to customize the size of the paper and enter in there your custom size (make sure milimeters are selected as a unit).

I don't have Mac but I would assume word has an option to format pictures. You'll have to make sure that the picture used as a background doesn't have in its properties a border added. Again, this is a long-shot advice since I don't have a Mac.

I'm not sure I understand exactly what you need, but if you want to edit a PDF file after it was created with doPDF, you cannot do it. If you want to have underlined text in the resulting pdf, you'll have to underline the original text in the file you first converted from.

Can you send a screenshot to info[]dopdf[]com so I could add a possible bug report?

If you're using the same folder to save pdf files most of the times, you could use the Always use this folder option to avoid that 2nd save window.
